COLLECTION NOTICE

The health information collected through your intake forms, clinical records, and online booking at Nurturance Health is collected under the authority of section 20(b) of the Health Information Act (HIA), for the following purposes identified in section 27 of the Act:

• Providing health services
• Determining or verifying your eligibility to receive a health service (including processing insurance claims through TELUS Health and Alberta Blue Cross)
• Conducting investigations, discipline proceedings, practice visits, or inspections relating to members of a health profession or health discipline (such as reviews conducted by the College of Physiotherapists of Alberta)
• Conducting research or performing data matching or other services to facilitate another person’s research
• Providing for health services provider education
• Carrying out any purpose authorized by an enactment of Alberta or Canada
• Internal management purposes, including planning, resource allocation, policy development, quality improvement, monitoring, audit, evaluation, reporting, and obtaining or processing payment for health services

Automated systems: The health information collected through your clinical records at Nurturance Health is input into Heidi, an AI-powered clinical documentation (scribe) tool, for the purpose of generating your clinical notes.

We collect the least amount of health information necessary to fulfill these purposes, and only collect health information directly from you unless you are unable to provide it or authorize us to collect it from another source.

The confidentiality and privacy of the health information collected by Nurturance Health is protected by the provisions of the Health Information Act and the safeguards described in our Privacy Charter.
